FREQUENTLY ASKED QUESTIONS
Why choose us?
We are a team of physician-scientists trying to have the highest possible impact on the health of our patients, which means focusing on prevention, taking into account all possibly informative data sources, and providing you with the time and access to internalize and implement your treatment plan. We are focused on not just providing the best precision healthcare available, but to use our research prowess to define the future of it as well.
Our backgrounds are pretty unique. We don’t know of many other physicians that simultaneously have benchside experience spanning imagining single RNA transcripts in cells to explore epigenetic differences and managing 20 critically ill-intubated patients in a COVID ICU, who trained at some of the most prestigious hospitals. Our emergency department background means that you can trust us to handle any unanticipated problem and this breadth of experience gives you a uniquely informed perspective that you can trust.
We are committed to delivering exceptional care to our patients, translating the full breadth of the available science, to help you live your best possible life.
Do you accept insurance?
We all know the many middle-men in healthcare cause more problems than they help solve. The current insurance as payor paradigm is cumbersome and stifles innovation in healthcare. Insurance providers only reimburse a set catalogue of billing codes under a fee-for-service model with fixed rates. Under these contracts, a physician cannot bill separately for non-covered practices or bill the difference to the patient. This means a physician cannot try to innovate without overhauling the entire insurance landscape.
Moreover, the fixed fees that insurers offer mean physicians can only compete on volume of patients seen rather than quality of care provided. This has forced most physicians to shorten appointments to fifteen minutes or less, often seeing 30-40 patients a day! Under these constraints, a physician can only focus on the most glaring issues and offer streamlined, standardized treatment plans.
Free from these constraints, we are free to provide as much as time needed for each patient. We are free to innovate on services, dive deep into research, and fundamentally redesign healthcare from the ground up with platforms like integrative AI.
We are starting with a bespoke care model that gives unlimited access to our expertise to design individualized care plans, but our goal over time is to streamline our process and learnings, and hopefully, with AI, be able to provide widely accessible care at an affordable price point.
Where is Precision Longevity located?
Precision Longevity is primarily a telemedicine practice, enabling us to serve clients conveniently in Texas, California, Florida, and Massachusetts. Our practitioners are located in Austin, TX (Michael Loesche, MD, PhD) and Boston, MA (Paul Ginart, MD, PhD).
While we don't have a physical office, we can arrange in-person visits or consultations at your home or office in Austin and Boston on a case-by-case basis.
Why a focus on prevention?
There is truth in the expression, “a stitch in time saves nine.” Based on many years of experience treating emergencies in some of the busiest hospitals, we saw how an increasing number of patients were presenting due to chronic conditions that should have been addressed years to decades earlier with few solutions available at the moment. This prompted us to do a deep dive into the science of longevity. After deeply reviewing and investigating the science, we realized that modern medicine has performed miracles in sickcare, but has largely neglected healthcare. We have decided to focus on the latter – healthspan over lifespan.
Preventative care has the largest effect size, compounding over the course of your life. By starting early, measuring often, and tailoring a plan specific to you, we believe we can achieve the best results possible. This doesn’t mean that we don’t fix or manage existing issues you might have, and in fact, we believe that one of our unique values is to be thoughtful about integrating treatment plans for your existing chronic disease with the best preventative practices. By addressing risk early, we reduce unnecessary procedures, hospital visits, and complications, ultimately improving your quality of life, giving you a return on your most important asset- your time, health, and energy.
How do you personalize care?
There is a central tension between modern medicine’s drive to practice “Evidence Based Medicine” and the idea of a physician carefully individualizing treatment plans to fit the specific patient in front of them.
For the former, large clinical trials enroll 100s to 1000s of patients, looking for population level effect sizes, and inherently assuming people are interchangeable. On the other hand, we are treating individual patients not populations, and each patient should be considered individually. To practice personalized medicine, we focus on your personal traits, and adjust treatment plans based on your unique biology and environment. Doing this correctly requires a deep understanding of biology, the scientific process, as well as clinical practice.
Some recommendations in personalized medicine are not and cannot be proven under that standard, so we rigorously research and assess the risks and benefits associated with different therapies and together decide on a treatment plan that works for you. We approach our care with the N of 1 clinical trial paradigm- carefully analyzing your biomarkers before and after interventions to see what is working well and how to modify accordingly.
Why do we favor measuring more than less?
Some physicians advocate measuring less rather than more, because they find themselves uncomfortable with data that doesn’t have obvious significance. The real challenge is in identifying the strength of the association between a piece of data and the outcome of interest. We believe in harnessing as much data as we can, in a cost-effective way, to obtain as complete a picture as possible of your health.
Truly personalized care needs to consider the significant variability found between individuals. This can only be done by integrating larger volumes and more complex sets of data. This complexity can be intimidating for most physicians, but as experienced physician-scientists with strong backgrounds in data science and machine learning, we see this as an opportunity.
In data science, you will come across two complimentary ideas. The first, “garbage in - garbage out”, says it is critical that all data being used to make decisions must be of high quality. Second, “a thousand weak predictors makes a good one”. This mantra is demonstrated by the near psychic abilities of advertisers to identify who you are or what you want based on web browsing experiences.
We apply both philosophies to our practice. We gather the high-grade medical data where possible and lean on the massive amounts of data that less precise sources such as wearables can bring to the table. Using each set of data responsibly allows us to do much more than with any isolated piece of information.
We recognize that for some people, information without clear significance can just breed anxiety and ultimately be detrimental. This is why, after our personalized intake process, we are careful to really get to know you and your goals before recommending a plan that works best for you.
What does a membership include?
A membership includes being our patient, with full access to us. We’ll do a thorough personalized intake going over your medical history, as well as determining your goals and challenges. Afterwards, we’ll craft a personalized care plan, manage your medications, and check in with you at regular intervals. We’ll also be available to you 24/7 to talk should emergencies arise. Our intake includes genetic testing, and an extensive biomarker analysis tailored to fit your goals.
We will be your physicians, but you should think about us as your health consultants. We provide our expertise, and will help you navigate the system. Our services cover:
Data Gathering - We will help you get all of your medical records from previous providers including visits, lab work, and imaging. We work with easily available lab providers such as Quest Diagnostics as well as specialized labs such as Boston Heart Diagnostics where needed. We will also work with your local imaging centers to get any other testing that is needed. We also help you find local providers for ancillary testing such as DEXA scans and VO2 max testing.
Interpretation of Data - We will go through your medical data such as blood work and imaging, but also genetic information, wearables, family history, and ancillary testing. In addition to personal review, our AI research assistant provides a second-eye to help aggregate these large sums of data. We then go over the findings to tell you where you are ahead of the curve and where you need to focus your efforts.
Development of personalized plan - We will give you recommendations to improve your fitness, sleep, stress, diet, and prioritize your efforts. Depending on your medical history, different prescription medications may be used such as lipid lowering medications, GLP1 inhibitors, metformin, rapamycin, hormonal replacement therapy, and tailored supplements. We only use these tools strategically to optimize your health when it fits your individual risk-benefit profile.
Your membership includes our time and expertise. We do not cover the other costs such as labs, imaging, and medications. While we will work diligently to have as many of these 3rd party services go through your insurance, some testing or treatments may not be covered by your specific plan. We will always incorporate expected costs of these decisions into your individualized plan.
We are planning on building out slowly and have limited intake ability available. If you are interested in joining, reach out to schedule an introductory call to see if we are a good fit.
What is your opinion on longevity influencers?
We believe that knowledge is power. The problem is filtering the knowledge from the noise. We believe that the vast majority of these influences are well meaning and largely helpful. However, while bringing awareness to health and wellness, some of these influencers have espoused false information that is sometimes harmless, but occasionally dangerous. Our job is to help our patients navigate the ever-growing sea of information on optimal healthcare so that we can make the best decisions for them, together.
How do you view the role of AI in healthcare/your practice?
As healthcare technologists, we are frustrated by existing healthcare IT. We are big believers in AI and believe that it will completely transform healthcare, for the benefit of patients and physicians alike.
There was a time when a physician could conceivably integrate all medical data points available to them holistically. In fact, clinical guidelines are often made with simplicity at the top of mind, so that humans can memorize and apply them readily. Today, the vast amount of testing ranging from blood work to genetics can overwhelm even the best physician. This is where AI-systems actually do very well, in synthesizing and summarizing vast amounts of data. We ensure that any AI tool we use complies with HIPAA and other privacy regulations, so your sensitive information remains confidential. Incorporating these tools into our approach will allow us to finally deliver on the promise of truly personalized medicine in a way that would otherwise be impossible to do. We are clearly biased, but we think clinicians who are also data scientists are the future in an AI-enabled world.
As such, we are looking to incorporate AI as much as possible into our practice, while maintaining your data privacy needs at the forefront. We currently operate within existing solutions, but part of our work is exploratory, building AI agents that streamline our clinical practice. Much of the data we are interested in comes from different sources, including wearables, continuous glucose monitors, in addition to the more traditional data like labs and imaging. We believe AI-agents will integrate all of this data effectively, and are prototyping our own internal versions.
